The joining process queues up state forms.
This will allow you to book sessions at Shrooms Cafe for one full year
After one year you can easily renew.
Do it online
1. Start by clicking on "Book Now" below to select a time for your Shrooms Cafe preparation session.
2. Create an account with us by clicking the sign up button located at the bottom of the page.
3. Reselect your desired session from one of our 15 minute time slots labeled "Shrooms Preparation Session".
4. Fill out all forms provided. Estimated 30 minutes.
5. Facilitator will call you via phone for your 1:1 session.

The sign up process requires the informed consent, client bill of rights, client information form, authorization to share de-identified information, and notice of opt-out of disclosure of de-identified data , & client transportation plan.
