Sign-up process & what to expect
After providing your contact information, you will receive 5 separate emails.
These emails will include the links to the 5 forms that are needed to complete your signup.
After you open each form, you will be able to Docusign them.
Once all forms are "marked completed", you will receive a call from your facilitator to review and complete your sign up.
24 hours after this review, you are able to book your first session.
This signup process is only required on your first visit - the intake paperwork is valid for one year
The forms include:
Informed Consent - Informs you of all your rights and clearly explains your privileges as a client
Client Information Form - Provides your facilitator with any health information that may help your facilitator best support you
Transportation plan - You must have a plan to leave the cafe that doesn't involve you driving a car or riding anything with wheels
Safety & Support Plan - Lets us know who to call in case of emergency and addresses any concerns you may have
303 Data Form - Unidentified demographics information that you can choose to share, or not
