Sign Up for Shrooms Cafe
It's free to join. It takes just a few moments!
The state of Oregon requires you to fill out paperwork and review with a facilitator 24 hours before your first session. After initial sign up you will be able to book sessions the same day without this extra step.
Do it online
1. Start by clicking "Join" below to select a time for your Shrooms Cafe preparation call.
2. Create an account by clicking the sign up button located at the bottom of the page.
3. Reselect your desired call time from one of our 10 minute time slots by clicking "Join".
4. Fill out all forms provided during booking. Estimated 20 minutes.
5. Facilitator will call you via phone to complete your sign up.

The sign up process requires the informed consent, client bill of rights, client information form, authorization to share de-identified information, and notice of opt-out of disclosure of de-identified data , & client transportation plan.
The sign in process is intended to give you a chance to ask any questions about the forms and sessions.
